It is reported that 1 in 4 Americans age 65+ fall each year. Millions of these seniors are treated for fall-related injuries including death. Silver Sitters is committed to assisting in reducing falls. Our caregivers anticipate the needs of our clients to decrease the risk of falling.
Often the Senior needing the care may not be the individual who falls. Primary caregivers (specifically spouses) are not trained in lifting and transferring. Untrained individuals assisting with ADL care can cause harm and injury to not only themselves but the client. Let our trained professionals step in and reduce this statistic.
Care Coordination is available for all our Silver Sitters Clients. Our experienced Care Managers will assist in streamlining care including:
Gathering information from all of your healthcare providers
Developing a Client-Centered care plan
Medication review and management including ensuring ample medication supply available
Scheduling needed appointments with care providers including assisting with the set up of virtual Telehealth visits
Emergency management planning and preparation
Incontinence ordering and supply management
Assisting with walking from bed to wheelchair
Bathing, dressing and grooming assistance
Safety and fall prevention
Status reporting to family
Toileting and incontinence care
Meal preparation and nutrition
Laundry and change of bed linens
Grocery shopping and errands
Transportation to social & recreational activities
Assistance with light exercise
